Description

This position is responsible for providing UT Health San Antonio study participants with education, support, and navigation services that are tailored to their identified needs. The candidate will be responsible for ensuring that participants receive safe, quality, and valuable, participant-centered support by collaborating with members of the Checkmate: Fighting Colorectal Cancer through Screening and Avanzando Equidad de Salud teams.



Responsibilities

  • Establish relationships with members of the study team and external community organizations.

  • Outreach to and assess participants needs, provided appropriate resource materials, and provide navigation services. 

  • Document detailed outreach notes in participants electronic medical record and in REDCap.

  • Perform outreach calls, engage in face-to-face encounters, and perform home visits when necessary (i.e., patient unable to get to clinic).

  • Establish trusting relationships with participants while providing general support and encouragement.

  • Discuss changes in participants identified needs with members of the study team.

  • Participate in team meetings and other meetings as scheduled.

  • Perform other duties as designated by the study team.



Qualifications

  • Ability to communicate and interact with diverse levels of individuals. 

  • Ability to work as part of a team or individually while meeting deadlines in a multi-tasking environment. 

  • Bilingual (English and Spanish) with high proficiency in spoken and written Spanish.

  • Excellent time management, organizational and interpersonal skills.

  • Experience engaging with clinical teams.

  • Strong organizational skills, attention to detail and accuracy.

  • Strong problem-solving skills.


EDUCATION:



  • High School Diploma or GED is required.


EXPERIENCE:



  • One (1) year of related experience is required.

  • Bilingual (English and Spanish) is highly preferred.


LICENSE AND CERTIFICATION:



  • Successful completion of a Community Health Worker formal training program such as from a college or other education institution is preferred.
